As a reminder, Health Plan launched a Medicare Advantage Dual Special Needs Plan(D-SNP) starting 01/01/2026. Our plan marketing name is Health Plan of San Joaquin/ Mountain Valley Health Plan Advantage D-SNP (HMO). This new plan serves members who are eligible for both Medicare and Medi-Cal known also as Medi-Medi members.

As part of the initial phase of plan implementation, we would like to provide clarification regarding plan name references and how it may appear within third party portals such as Noridian or the Department of Health Care Services (DHCS) Automated Eligibility Verification System or other communications.

Initially, you may have encountered our insurer information listed as San Joaquin County Health Commission which is our legal name, or Advantage D-SNP which is our plan benefit package name. We would like to clarify that we are working with Noridian and the DHCS to update this information to align to our plan marketing name to minimize any confusion.

We are also aware of some initial references to “Health Plan Commission Advantage”. As noted above our plan marketing name is Health Plan of San Joaquin/ Mountain Valley Health Advantage D-SNP(HMO). Please keep this in mind when providing care and ensure to serve eligible HPSJ/MVHP members accordingly.
