About the plan
Advantage D-SNP combines the benefits of Medicare and Medi-Cal into one plan that provides you with the care you deserve.
Here’s how:
Advantage D-SNP contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees who live in Alpine, El Dorado, San Joaquin and Stanislaus counties.
Who is eligible?
- 21 and older
- Have both Medicare Part A and Medicare Part B
- Are currently eligible for Medi-Cal
- Live in Alpine, El Dorado, San Joaquin and Stanislaus Counties
What do I get?
Access to doctors, hospitals, pharmacies, providers of Long-Term Services and Supports (LTSS), and other providers.
Care coordinators to help you manage all your providers and services and supports. They all work together to provide the care you need.
A care team that will work with you to develop a care plan that addresses your health and support needs- a plan that reflects your personal preferences and goals.
What do I pay?
Premiums $0
Because you have Medi-Cal, you won’t pay any monthly premiums, including your Medicare Part B premium, for your health coverage.
Deductibles $0
You don’t pay deductibles in Health Plan of San Joaquin/Mountain Valley Health Plan Advantage D-SNP.
Maximum out of pocket cost $0
There’s no cost sharing for medical services in Health Plan of San Joaquin/Mountain Valley Health Plan Advantage D-SNP, so your annual out-of-pocket costs will be $0.
