Health Plan is proud to announce its investment of $100 million in a three-year Community Reinvestment Program. The following grant opportunities are included:

  • Health Information Exchange (HIE) Grants Incentive Program
  • Doula/Community Health Worker Recruitment Workforce (DCHWR)
  • Provider Recruitment Grant (separate from Community Reinvestment Program)
  1. HIE Grants Incentive Program

Health Plan has earmarked $8.775 million in Community Reinvestment funds to bolster data sharing initiatives aimed at strengthening the connection between healthcare providers and the Health Plan-supported Health Information Exchange (HIE). These funds are designed to increase healthcare provider utilization of the Health Plan-supported HIE or Standard Data Sharing Templates for exchange of patient data for treatment, payment and operations.

Below are the grant tiers that traditional (clinical-based) and non-traditional (Community-Based Organizations or county-based) providers who render services for Health Plan members can request funding for.

  • Basic HIE
  • Excel Data Sets
  • Expanded HIE
  • Enhanced HIE

  1. Doula/Community Health Worker Recruitment Workforce (DCHWR) Grant

Health Plan has earmarked $7 million in Community Reinvestment funds to support organizations and individuals dedicated to providing direct services to Health Plan members. With a focus on supporting high-quality care, the program aims to provide resources and incentives for the recruitment and retention of these essential healthcare professionals via their prospective employers.

Eligible Positions:

  • Certified Community Health Worker
  • Certified Doula
  • Certified Substance Use Disorder Counselor

Eligible Entities:

  • Organization must be categorized as a Federally Qualified Health Center (FQHC), Health Plan credentialed provider/medical group, a community-based organization with a 501(c)(3) designation, or a Hospital Emergency Department.
  • Must be contracted with Health Plan/Medi-Cal network.
  • All applicable positions must be credentialed by Health Plan prior to receiving grant fund.

Maximum Funding Amount: $70,000 for one year per eligible position.

  1. Provider Recruitment Grant

The Provider Recruitment Grant supports recruitment efforts to expand access to care for Health Plan members.

Grant Requirements:

  • Be a brand-new provider to the Health Plan network.
  • Provide healthcare services at a professional office or clinic within the Health Plan service areas (San Joaquin, Stanislaus, El Dorado and Alpine Counties).
  • Be a licensed practitioner.
  • Be Medi-Cal enrolled as an Ordering, Referring, and Prescribing (ORP) practitioner.
  • Meet and complete all credentialing requirements.
  • Employed as full-time or part-time.
  • Maintain good standing for the entire grant term.

Program Structure: A Memorandum of Understanding (MOU) must be executed between Health Plan and the hiring entity. The hiring entity submits a grant application.

Payment Structure: Payment is divided into two (2) equal distributions.

  • Successful end of program/employment anniversary year (PY) 1 of employment: 50%
  • Successful end of program/employment anniversary year (PY) 2 of employment: remaining 50%
Provider Type Total Amount
Hard to recruit specialist MD/DO includes:

  • Plastic surgery, reconstructive surgery
  • Endocrinology
  • Rheumatology
  • Oncology – Hematology
  • Psychiatry
  • Neurology
$150,000 grant funding
All other specialists MD/DO (e.g., OBGYN) $125,000 grant funding
PCP (MD/DO) $100,000 grant funding
Certified Physician Assistant (CPA)/Physician Assistant (PA)/Nurse Practitioner (NP)/Marriage and Family Therapist (MFT)/Licensed Clinical Social Worker (LCSW)/Substance Abuse Counselor/Behavioral Health Counselor/Certified Midwife (CNA) $50,000 grant funding
