Health Plan expanded its operations effective January 1, 2024. As a result, member identification (ID) cards have been updated to include dual branding for Health Plan. Below is a sample of the dual branded ID card.

As a reminder, providers may verify Health Plan member eligibility through the following resources:

  • Automated Eligibility Verification System (AEVS): Via phone: 1-800-456-2387,1-800-866-2387 or Via Medi-Cal website: https://secure.medical.ca.gov/mcwebpub/Login.aspx
  • Online at www.hpsj-mvhp.org via DRE, our secure provider portal
  • Customer Service: (209) 942-6320 (Monday – Friday, 8am 5pm)
  • Interactive Voice Response (IVR): (209) 942-6303 (Available 24/7)
    Options in accessing member eligibility and primary care provider (PCP) status:
    1. Say the Patient’s ID number/ MCL CIN#/ SSI#
    2. Use telephone keypad to enter the Patient ID number

    Receive a confirmation number with each call
