The Department of Health Care Services (DHCS) has informed HPSJ that at this time:
- PM 160 Encounter forms will only be eliminated from the state Medi-Cal program
- All Medi-Cal Managed Care Health Plans, including Health Plan of San Joaquin, are still required to submit these forms
