The following is a reminder of Controlling High Blood Pressure (CBP). Patients 18 – 85 years of age with a diagnosis of hypertension (HTN) who have blood pressure (BP) monitoring throughout the year and have a controlled blood pressure indicated by a reading of less than 140/90 mm Hg.
Recommended Best Practices
- Perform 2 BP checks at every HTN patient visit.
- If BP reading is high during visit retake at the end of the visit.
- Blood Pressure Monitors to HTN patients who meet criteria (authorization may be required for certain monitors and cuffs).
- Document member reported BP taken from an electronic cuff during telehealth visits.
- Monitor and follow up with members who are not adequately controlled during the year.
- Offer drop in BP checks at your office to patients with HTN – record BP results from all drop ins in patients’ medical record.
