The following is a reminder of Controlling High Blood Pressure (CBP). Patients 18 – 85 years of age with a diagnosis of hypertension (HTN) who have blood pressure (BP) monitoring throughout the year and have a controlled blood pressure indicated by a reading of less than 140/90 mm Hg.

Recommended Best Practices

  1. Perform 2 BP checks at every HTN patient visit.
  2. If BP reading is high during visit retake at the end of the visit.
  3. Blood Pressure Monitors to HTN patients who meet criteria (authorization may be required for certain monitors and cuffs).
  4. Document member reported BP taken from an electronic cuff during telehealth visits.
  5. Monitor and follow up with members who are not adequately controlled during the year.
  6. Offer drop in BP checks at your office to patients with HTN – record BP results from all drop ins in patients’ medical record.
