The Centers for Medicare & Medicaid Services (CMS) announced that the Office of Management and Budget (OMB) has approved the updated Medicare Outpatient Observation Notice (MOON) for 3 years. The new version is posted in English and Spanish on CMS’ Beneficiary Notices Initiative (BNI) webpage at https://www.cms.gov/medicare/forms-notices/beneficiary-notices-initiative and Health Plan’s website under Advantage D-SNP Forms at https://www.hpsj.com/forms-documents/.

• No changes were made to the MOON delivery or timeframe requirements.
• CMS made readability and design improvements for consistency with other Medicare notices.
• The updated MOON is effective now and expires February 28, 2029.
• Providers who have existing stock of the expired MOON may continue to use the expired version until April 20, 2026, but must transition to the updated form no later than that date.

Providers are required to deliver a MOON to Medicare members (including Health Plan Medicare Advantage D-SNP members) informing them that they are outpatients receiving observation services and are not inpatients of a hospital or critical access hospital (CAH).
