Health Plan will be performing planned system maintenance to support the implementation of the new Social Care Connect module within JIVA, the authorization system
Beginning Monday, July 13, 2026, the new Social Care Connect module will be available. Social Care Connect is intended for Health Plan’s contracted community support providers, Enhanced Care Management (ECM) providers, primary care providers (PCPs), community-based organizations (CBOs), physician groups, and more entities within our network to create and track referrals, known as “closed loop referrals”.
This enhancement supports improved coordination of social care referrals and related workflows. Following this implementation, ALL Enhanced Care Management (ECM) and Community Supports requests will need to be entered through the Social Care Connect module.
Scheduled Maintenance Window:
- 8:00pm Friday, July 10, 2026 through 12:00pm Saturday, July 11, 2026
During this maintenance window, JIVA will be unavailable while system updates are completed.
