We are sending this alert because your patients may have filled a prescription for Phenytoin 125mg/5ml suspension within the last six months (180 days).
The reason for the recall is that product from these two lots of Phenytoin Oral Suspension may not re-suspend when shaken, as instructed for administration, which could result in under or overdosing.
HPSJ is now contacting HPSJ members who have filled a prescription for Phenytoin 125mg/5ml suspension within the last six months (180 days). We are recommending they contact their pharmacy to check to see if their medication is part of the recall.
