Health Plan of San Joaquin (HPSJ) is excited to share a new Medi-Cal program that can transform your practice to better serve your Medi-Cal patients.
Please join us on July 19, 2023, from noon to 1:30 pm to learn about the new Medi-Cal program for primary care practices. This is a unique opportunity for grant funding that can be invested into people, processes, and tools focused on improving quality and value for your patients. Examples include but are not limited to:
• Upgrading your Electronic Health Record (EHR) system with population health management tools
• Adopting of Admit, Discharge, and Transfer (ADT) feeds to improve care coordination and patient safety
• Improving patient engagement with effective strategies for assigned but not seen populations
• Implementing Social Determinants of Health (SDOH) screening and referral tools
• Starting a group prenatal care program and/or hiring doulas
• Developing an integrated behavioral health model
• Preparing for value-based payment models
In this Look and Learn, HPSJ and RecastHealth will provide information on how the Equity and Practice Transformation Payments Program works and outline essential steps for partnering with us to apply.
This Look and Learn is intended for potential program participants which include
small- and medium-sized independent primary care practices specializing in:
• Pediatrics
• Internal medicine
• Family medicine
• Obstetrics and gynecology
• Behavioral health providers integrated into a primary care setting
This Look and Learn is NOT intended for large practices (over 50 providers) or practices affiliated with a healthcare system or Federally Qualified Health Center (FQHC). HPSJ is exploring other avenues to support practices that are not eligible for this program with similar transformation work.
