In accordance with All Plan Letter (APL) 23-023 and 23-027, effective January 1, 2024, all Medi-Cal Managed Care Plans (MCPs) will become responsible for the full Long-Term Care (LTC) benefit at the following Intermediate Care Facility for Developmentally Disabled (ICF/DD) facility types and homes:

  • Intermediate Care Facility for the Developmentally Disabled (ICF/DD)
  • Intermediate Care Facility for the Developmentally Disabled – Habilitative (ICF/DD-H)
  • Intermediate Care Facility for the Developmentally Disabled – Nursing (ICF/DD-N)
  • Subacute Care Facilities

HPSJ will issue new member identification cards before 1/1/2024 for members who reside in an Intermediate Care Facility (ICF) and Subacute Facility. The member identification card will state the following, ‘Plan: Medi-Cal’; however, when a provider verifies eligibility, the member will not be assigned to a Primary Care Provider (PCP); the member will be assigned to HPSJ as the PCP. This will not interrupt members’ care, and members can continue seeing their assigned providers prior to this change.
