(French Camp, CA) – In order to facilitate well care exams that help create a healthy community, Health Plan of San Joaquin is this summer engaging with providers, local partners and HPSJ members at public events with Caravan for Health (CFH).

CFH events include local baseball games with Stockton Ports and Modesto Nuts partnerships (flyers with details attached) and the 27th Annual Tracy African American Chamber Juneteenth Festival (June 11, click here for details), and more opportunities throughout the summer.

HPSJ is notifying their members they can make an appointment with their primary care physician or clinic to complete their well care visit or attend a local health fair. At these CFH summer  events, HPSJ members have the opportunity to receive FREE testing and health screenings, as well as health information and related materials that include women’s/men’s self-care items and other items that promote good health such as pins, masks and hand sanitizers.

For more information – including the CFH calendar of Health Fair Events – HPSJ members, providers, and community partners are invited to visit www.hpsj.com/care.
