REMINDER
Healthcare Effectiveness Data and Information Set (HEDIS) Medical Record Review (MRR) season is coming. Lists will be faxed out in early February 2024.
IMPORTANT: Please note, Health Plan is willing to assist with medical record procurement. Therefore, the plan does not pay medical record copy fees. Email HEDIS@hpsj.com if you require assistance with record collection.
RECORDS COLLECTION DEADLNE: Records received after MARCH 29, 2024, may not be processed.
All requested records should be faxed to (209) 762-4730 or mailed to 7751 South Manthey Road, French Camp, CA 95231-9802.
ADVISORY: This process is TIME SENSITIVE in preparation Health Plan recommends the following:
- Designate a contact person for the collection and/or coordination of records.
- If you have an Electronic Medical Record (EMR) system that allows for remote access, please contact the Health Plan team as soon as possible to coordinate set-up. Direct EMR capture is a no cost service for Health Plan contracted providers.
