HEALTHCARE EFFECTIVENESS DATA AND INFORMATION SET (HEDIS) MEDICAL
RECORD REVIEW SEASON IS COMING
HEDIS MEDICAL RECORD REVIEW SEASON IS COMING UP!
The first notification was sent on December 14th, 2021, and the second notice was sent
January 20th, 2022.
Medical Record Collection for HEDIS Hybrid measures will begin February 1, 2022.
RECORDS COLLECTION DEADLINE: March 30, 2022
PLEASE ADHERE TO THIS DEADLINE
HPSJ will be requesting medical records via fax from your offices. All requested records should be faxed to (209) 762-4730.
- Please designate a contact person for the collection of records, this process is TIME SENSITIVE.
- In rare exceptions when medical records need to be picked up from your facility; please contact HPSJ’s HEDIS Team when records are ready by having all records sorted by member and enclosed in folders to ensure member PHI is safeguarded.
- HPSJ will not pay for copy service fees. If records are requested, providers are
expected to provide all medical records being requested without fee.
