Formulary Update – FORA V30 Meters are replaced with FORA Premium V10 Meters

The FORA V30a Blood Glucose Monitors are being discontinued by the manufacturer. Therefore, they will be replaced with FORA Premium V10 Blood Glucose Monitors.
FORA V30a meters may still be available at some pharmacies, for that reason we will not remove this meter from HPSJ’s formulary.

What does this mean to providers and pharmacies?
Prescribe and dispense as usual. HPSJ will now cover the FORA Premium V10 Glucose Meter.
What does this mean to members that currently have a FORA V30 Meter?
Not a thing. Continue to use existing FORA V30 meters and test strips.
Will there be new test strips?
Continue to use/order the same FORA test strips.
