Dear Health Plan of San Joaquin Network Providers,

The Health Plan of San Joaquin (HPSJ) is committed to ensuring that our members continue to access quality care as we terminate Sutter Valley Hospitals from our provider network. Thank you for partnering to effectively serve HPSJ members through the transition.

Effective March 9, 2022, HPSJ’s provider network will not include Sutter Valley Hospitals. This termination applies to the following facilities:

• Memorial Medical Center Modesto
• Memorial Hospital Los Banos
• Sutter Tracy Community Hospital
• Sutter Medical Center Sacramento
• Stanislaus Surgical Hospital
• Sutter Infusion
• Sutter Home Health (Visiting Nurse Association-VNA)

Please note that authorizations for HPSJ members to receive services at Sutter Valley Hospitals will be approved through March 8, 2022. Following this date, such authorizations will expire.

HPSJ members scheduled for elective procedures at Sutter Valley Hospitals after March 8, 2022, should be redirected to a contracted facility if services cannot be rescheduled and provided before the effective termination date. HPSJ will partner with network providers as appropriate to ensure continuity of care for members receiving inpatient services at Sutter Valley Hospitals after March 8, 2022.

Network providers are encouraged to begin coordinating with the following network hospitals to ensure a smooth transition for HPSJ members with authorizations to receive services at Sutter Valley Hospitals:

• Dameron Hospital Association
• Doctor’s Hospital of Manteca
• Doctor’s Medical Center Modesto
• Emmanuel Medical Center
• Lodi Memorial Hospital Association
• Oak Valley District Hospital
• San Joaquin General Hospital
• St. Joseph’s Medical Center
