The HEDIS Integrated Reports (formerly called Gap in Care Reporting) are available!

Thanks to feedback from our provider partners, HPSJ is now delivering reports on
gaps in care that are easier to use and also illustrate gaps in care at multiple levels.

We’ve made gap in care reports easier and even more useful for providers.

• Log into the secure DRE portal as normal.
• Navigate to the HEDIS Integrated Reports tab to locate your gap in care
  reporting for HPSJ patients.
• Reports are available at these multiple levels:
  • Medical Group/FQHC
  • Clinic Site or Individual provider
  • Individual patient
