On August 30, the California Department of Health Care Services (DHCS) launched the Equity and Practice Transformation (EPT) Provider Directed Payments Program. This program will provide primary care practices the opportunity to obtain funding for practice transformation activities aimed at advancing health equity and population health.
In a recent webinar, DHCS announced that applications are due October 23, 2023, 11:59 pm. DHCS also provided information, including program activities, funding allocations, and application questions. Maximum payment levels range from $375,000 for 500 to 1,000 members to $10 million for over 100,000 covered lives.

We strongly recommend completing your program application by October 16, 2023.
