As the roll-out of COVID-19 vaccinations continues, HPSJ is working with San Joaquin County Public Health Services (SJCPHS) and the county Medical Society to make sure all local practice staff who wish to be vaccinated have timely access to vaccines.
Following is a message from (SJCPHS), along with the link to their brief survey.
Please fill out this survey from San Joaquin County Public Health Services to collect contact information and assess vaccination needs for your practice. This survey will help county Public Health coordinate COVID-19 vaccine distribution.

The deadline to complete this survey is now extended, but please fill out ASAP.
For those who want vaccine for staff only, we currently are working to finalize agreements with hospitals and pharmacies to vaccinate our healthcare providers. If you complete this survey, we can reach out with next steps.
