We’re excited to announce Health Plan of San Joaquin’s new Disease Management program for HPSJ patients with COPD (chronic obstructive pulmonary disease)! We offer telephonic Disease Management services to all eligible members. Our nurse case managers and social workers can help your patient manage their chronic conditions and get access to vital services such as transportation and in-home support.

Services from the HPSJ Prenatal Health Education Program include:
- Medication management
- Goal setting for treatment of chronic conditions
- Transportation to appointments
- Access to health education materials
Our Disease Management team can get your patients connected with resources. We want to help you provide guidance towards better self-management of chronic conditions. Other Disease Management programs include diabetes, asthma, and congestive heart failure.
