February is American Heart Month!
HPSJ is proud to sponsor Check. Change. Control., a community health education virtual class brought to you by the American Heart Association.
Check. Change. Control. is an evidence-based hypertension management program that utilizes self-monitoring blood pressure tools and lifestyle change education to empower participants to take ownership of their cardiovascular health.
This virtual, online-based class is a 4-week educational opportunity offered in English and Spanish. Participants will learn about hypertension, healthy eating, physical activity, stress management and more.
Everyone interested is invited! We ask that providers who refer their patients to these classes ensure that their patients have a blood pressure cuff ready for their first session. Blood pressure cuffs are a covered benefit for HPSJ members with a diagnosis of hypertension and can be ordered for patients through DRE.
