Healthcare Effectiveness Data and Information Set (HEDIS) Measure for 2022

The following is a reminder regarding the Childhood Immunizations Status (CIS) Combo-10.

Description

Vaccination must occur on or before child’s 2nd birthday:

  • 4 (DTaP) diphtheria, tetanus, and acellular pertussis
  • 3 (IPV) polio
  • 3 (Hib) haemophilus influenza type B
  • 3 (Hep B) hepatitis B
  • 4 (PCV) pneumococcal conjugates
  • 1 (Hep A) hepatitis A
  • 2 or 3 dose (RV) rotavirus -RV1 or Rotarix versus RV5 or Rotateq; it is important to specify which vaccine was given for rotavirus. If we are unable to determine which one was given, it will negatively impact the annual hybrid reviews.
  • 1 (MMR) measles, mumps, and rubella
  • 1 (VZV) chicken pox
  • 2 (Flu) influenza vaccines

Please ensure that all refusals are documented along with any education provided to the family.
