Effective: Retroactively to January 1, 2014
Code V5264 used for ear molds, which are needed in conjunction with hearing aids, no longer require Prior Authorization.
- Prior Authorization continues to be a requirement for hearing aids.
- Denied ear mold services for the period starting January 1, 2014 will be adjusted for payment.
