Provider Alerts

Adventist Health issued a notice to terminate their contract with Health Plan, effective July 1, 2025. Health Plan is working with Adventist to avoid contract termination. If a termination occurs, the following hospitals and provider groups will be affected:

• Adventist Health Lodi Memorial Hospital
• Adventist Health Lodi Memorial Hospital Clinics (physician group)
• Adventist Health Physicians Network (physician group)
• Adventist Health Home Care

Providers should begin by referring our members to other hospitals and specialty clinics available through our contracted network. Below is a list of contracted hospitals in our four service counties:

• Barton Memorial Hospital
• Dameron Hospital Association
• Doctors Hospital of Manteca
• Doctors Medical Center Modesto
• Emanuel Medical Center
• Marshall Medical Center
• Memorial Medical Center Modesto
• Oak Valley Hospital District
• San Joaquin General Hospital
• Joseph’s Medical Center
• Sutter Tracy Community Hospital
