As we prepare to go-live with Advantage D-SNP as a licensed plan for Centers for Medicare & Medicaid Services (CMS) effective January 1, 2026,
network providers are required to complete Model of Care training by November 30, 2025.
This requirement applies to all network providers who:
- Provide healthcare services to Dual Eligible Special Needs Plan (D-SNP) members, or
- Administer D-SNP healthcare benefits
Complete the MOC training available on our website: https://www.hpsj.com/provider-trainings/
If you have completed this training with another health plan, you must attest to completing the training through another source and provide proof of training. Proof can be a certificate of completion, training program outline, or web link to the training.
Instructions: After reviewing the presentation, please complete the Annual Network Provider Model of Care Training Acknowledgement & Attestation acknowledging you have completed the training.* Please fill out all required fields. One attestation form may be completed for multiple providers within one practice/clinic by listing each provider and National Provider Identifier (NPI).
