Why?
Since July 1, 2016, new regulatory standards have been in place for plans to capture and maintain accurate provider data from all of their network providers. Our goal is to finish this required task through our provider/HPSJ partnerships – and to do this as efficiently as possible, as we know how busy your facility practice is.
To help your facility practice submit the required information before these regulatory standards go into effect, HPSJ is making contact with you and your facility practice. Please complete and return the attached form by one of these methods:
1. Fax – One of the easiest ways to provide this information; just complete the attached form and return by December 15, 2016, to this secure HPSJ fax: 209-461-2565.
2. Email – Send an email with the form attached to providerservices@hspj.com.
