Our HPSJ network providers are important partners in delivering care to our members. As we continue to see ways to streamline processes, HPSJ has simplified the PCP Selection Form process and requirements for your busy PCP practices.
- The form will now be available online via your secure DRE Provider Portal
- The member information will be auto-populated
- English form option only available at this time
- You will be able to verify online, real-time, if the form with your PCP request, was approved
or denied and why
Effective May 1, 2018, all PCP Selection Form requests will need to be submitted via the DRE Provider Portal. Your Provider Service Representative will be available to provide additional training or to give you access to DRE if needed, or you can always call our Customer Service Department at 209-942-6320.
