Effective August 9, 2026, Medi-Cal Rx will no longer cover Basaglar (Insulin Glargine). This drug will be removed from the Medi-Cal Rx Contract Drugs List (CDL).

  • Basaglar will deny at the pharmacy with Reject Code 606 – Brand Drug/Specific Labeler Code Required.
  • CDL covered alternative is Lantus (insulin glargine) or insulin glargine-yfgn.
  • Prescribers may submit a prior authorization to Medi-Cal Rx with medical justification for the consideration for use of Basaglar.
