Overview: Providing Access and Transforming Health (PATH) supports CalAIM by offering free tools, training, and technical assistance that help providers strengthen Enhanced Care Management (ECM) and Community Supports. These resources help practices coordinate care more efficiently, streamline referrals, reduce administrative work, and build sustainable workflows. Providers can use PATH materials to train staff, improve communication with Medi-Cal health plans, and support smoother care transitions. For members, this means more consistent follow-up, better access to services such as housing and behavioral health, and improved overall health outcomes.
