On September 24, 2025 Health Plan of San Joaquin/Mountain Valley Health Plan (Health Plan) identified some claim adjustments were made which did not include the complete information submitted on the original claim.
Due to the incomplete information, you may see claims on your remittance advice for check date 09/26/25 with missing information and claim denials.
We have identified the impacted claims and will be making the appropriate corrections.
Please do not submit Provider Dispute Resolution requests or Corrected Claims for these issues.
