This is a reminder that the annual Provider Satisfaction Survey is currently in progress and is being administered by our third-party vendor, Press Ganey. The survey began on October 27, 2025, and will conclude on December 5, 2025. You may complete the survey by mail, online, or by phone.

The survey questions are distributed by mail and include instructions for responding by mail or online. If a response is not received, Press Ganey will follow up with a phone call from 1-800-588-1659 to conduct the survey.

We encourage you to participate, as your feedback is vital in helping us evaluate and improve the services we provide. Your responses are confidential, and data will be aggregated before being provided to Health Plan for review and improvement opportunities.
