Join the Conexiones Primary Care Access Project!
Health Plan is seeking a primary care clinic in San Joaquin or Stanislaus to join a funded initiative to improve access to care for Latino/a/x Medi-Cal members.
About the Project
The Conexiones team will partner with your clinic to design and test innovative solutions that address barriers Latino/a/x communities face in accessing quality care. Together, we’ll drive real, lasting change.
Why Participate?
• $75,000 grant to support your clinic’s involvement
• Work with human-centered design experts to co-create impactful solutions
• Improve healthcare access for historically underserved Medi-Cal members
What We’re Looking For
A primary care clinic ready to collaborate and innovate! In return, you’ll receive:
• Funding for your time and resources
• Hands-on support to test and implement solutions
• Opportunities to share insights and learn from other clinics
Commitment
• Project runs now through October 2026
• Estimated 2–10 hours per month
