About HPSJ
Health Plan of San Joaquin has been serving members enrolled in publicly-funded programs in the heart of the Central Valley since 1996. HPSJ is part of the Two-Plan model and serves as the local initiative in both San Joaquin and Stanislaus Counties. We have offices in French Camp and Modesto California.

- Health Information: You can speak to a HealthReach Advice Nurse by phone 24 hours a day. We also offer an audio-library to learn more about parenting, what to expect during pregnancy, and many other health related topics.

What is Medi-Cal?
Medi-Cal provides health care coverage and services for California residents that meet the eligibility requirements for one of the Medi-Cal programs. Medi-Cal programs cover a wide range of people based on age, disability, income, citizenship, and medical needs.
Who is eligible for Medi-Cal?
Medi-Cal covers low-income adults, families with children, pregnant women, adults 50 or older, persons with disabilities, children in foster care, former foster youth up to age 26 and young adults 25 years or younger regardless of immigration status
