  2. Continue growth, stabilization, and optimization of the D-SNP product.

Continuing the plan’s key role in the transformation of the Medi-Cal program, by bringing together provider partners, advocacy organizations, members, county leaders, community-based organizations (CBOs), and interested stakeholders to keep the community updated on all aspects of Medi-Cal and Medicare
    reforms, including policy and financing changes.
  2. Advance efforts across county agencies and community-based organizations to promote innovation and align funding streams and care coordination.
  3. Coordinate efforts to respond to the impacts of H.R. 1
    on the community at large.
