About Transitional Care Services (TCS)

Healthcare transitions occur when someone moves from a medical care facility back home or to a different care center. TCS is designed to ensure that qualified members receive the necessary support during these transitions.

The benefits of TCS include: helping members remain safely in their preferred home setting, ensuring patients attend and follow through with follow-up visits to maintain health and avoid hospital readmissions, educating patients about their health conditions, and connecting patients with resources for ongoing health and safety.

How can TCS Help?

TCS can help to make sure that a person who moves from one type of care to another gets the right support.

As a member, you can get help from a TCS Care Manager. This is someone from your health plan who will assist you during your transition of care.

With Transitional Care Services (TCS), our goal is to:

  • Help you stay safely in your preferred home setting.
  • Ensure you make and keep follow-up visits to stay healthy and prevent repeat visits to hospitals and emergency rooms.
  • Teach you about your health condition and how to manage it.
  • Connect you with the resources you need to stay healthy and safe.

Your TCS manager is here to help:

  • Understand your health needs
  • Schedule medical visits
  • Set up transportation for medical visits
  • Talk to you about your health plan benefits
  • Find community resources for housing, food, and more

Who can get TCS?

  • Anyone who has a health care transition can qualify for help from a TCS Care Manager. For example, a person who moves from a hospital setting to a care facility.
