Our locations in in the central valley serve our population in both counties. Health Plan of San Joaquin is part of the Two-Plan model and serves as the local initiative in both San Joaquin and Stanislaus Counties. We currently have offices in French Camp and Modesto California serving our communities.
San Joaquin County
French Camp is located in San Joaquin County, part of the Central Valley of California, and one of the original counties of the state, formed in 1850. With a 2010 Census population of 685,306, this is a dynamic, multi-ethnic and multi-cultural community situated along the San Joaquin Delta waterway that connects to the San Francisco Bay and the Sacramento and San Joaquin Rivers.
San Joaquin County is located 60 miles east of the San Francisco Bay area, 83 miles east of San Francisco, and 45 miles south of Sacramento.

Modesto is the 16th largest city in California with over 202,751 citizens.
