Road to Home: Long Term Care at Home
Partnering with our community based organizations and service providers. During this pandemic, there is a need to protect our most vulnerable Medi-Cal members who are discharged from the hospital but in need of long-term care services and supports (LTSS). Governor Newsom and our partners from the Department of Health Care Services (DHCS) are seeking ways to reduce this populations; risk of exposure to COVID-19 by providing opportunities for the many members that can safely receive care at home.
