Diabetes Prevention Program (DPP) Update

HPSJ is no longer offering DPP through Melon Health. We appreciate our members’ patience as we begin a new DPP program with Inspiring Communities!

Health Plan of San Joaquin is proud to offer the National Diabetes Prevention Program virtually in partnership with Inspiring Communities. Studies show that implementing simple lifestyle changes through this year- long program will help you lose 5 to 7 percent of your body weight which will reduce your risk of developing Type 2 diabetes by 58%.

By participating in our virtual National Diabetes Prevention program, members will connect with a health coach and access the program via browser or phone app. Throughout the year members are encouraged to lose 5 to 7 percent of their body weight through accessing weekly program modules, connecting with the online community, messaging, and calling their health coach, completing a daily health journal, and implementing regular physical activity. An Inspiring Communities Certified Lifestyle coach will help you every step of the way! Inspiring Communities is a CDC Fully Recognized Provider of the National Diabetes Prevention program.
