The Department of Health Care Services (DHCS) informed HPSJ that PM 160 CHDP encounter forms
were eliminated from the state Medi-Cal program effective July 1, 2017.
As a reminder –
All Medi-Cal Managed Care Health Plans are no longer required to accept PM 160 CHDP
encounter submissions from PCPs.
Health Plan of San Joaquin –
- Encourages electronic claims submissions (EDI) for a quicker turn-around time,
but also will accept CMS-1500 claim forms in place of PM 160
- If you are not currently submitting claims EDI, please contact our Provider
