How do I apply?

If you have qualified for Medi-Cal, you can request a plan choice form by
calling Member Services at 1-888-936-PLAN or locally at (209) 942-6320.

If you need help completing the forms you can contact a
Certified Application Assistor

If you qualify for Medi-Cal, you'll get a form in the mail to choose a health
plan. Just write "Health Plan of San Joaquin" in the box marked "Health Plan
Choice."
