Health Plan of San Joaquin offers a commercial insurance plan, Healthy
Connections to San Joaquin County In-Home Support Services (IHSS) workers.
Members who enroll in the Healthy Connections program receive medical and
vision services from their Primary Care Physician (PCP) who is selected at the
time of enrollment. As a Healthy Connections member, you have access to quality
medical care professionals who utilize most area hospitals. Some of the

What does it Cost?
You will only pay a premium of $10 per month for coverage. You will also have
copayments for some services, usually $5 or $10.
How do I apply?

In order to be eligible for Healthy Connections, you must work for two
consecutive months for which you were authorized to work an average of 60 hours
per month.

If you choose to enroll in Healthy Connections, you must submit your completed
application to the Public Authority by the 12th day of the month in order for
your coverage to start by the 1st of the following month.

IHSS workers can apply by contacting the San Joaquin County Public Authority
Office.

You must also work in San Joaquin County and live in an area that allows you to
access your PCP within 30 miles or 30 minutes.
