• Call the Healthy Families Program today at 1-866-848-9166 and give them your information so that you will receive updates!

• Health Plan of San Joaquin can help you fill out your annual eligibility renewal (AER).
• Call 1-888-896-7526 to get help with your renewal.

• HPSJ’s Enrollment Team can help you Monday – Friday, from 8:00 am – 5:00 pm call 1-888-896-PLAN (7526).
