Health Plan Providers:

We have scheduled a Physician Advisory Committee meeting on May 25th, wherein we will be conducting a focus group to discuss expansion of primary care capabilities (medical home, EHR), coordination with specialists, specialist’s medical home, and how HPSJ may assist you in providing services for your patients/our members. Please complete the survey and fax back to Provider Services at (209) 461-2565 before May 25th. (Circle all that apply.)

  1. I do not currently provide medical care for the SPD (Seniors & People with Disabilities) population and do not intend to.
  2. I currently serve this population and don’t need any additional assistance.
  3. I would like assistance from HPSJ to coordinate care and communication with specialists.
  4. I am interested in referring these members to HPSJ for Expanded Case Management.
  5. I am interested in coordinating care for HPSJ members with HPSJ Case Managers that come to my office.
  6. I am interested in coordinating care for HPSJ members with home visits facilitated by HPSJ ancillary providers (PAs, NPs.)
  7. I am interested in information/assistance with EHR (Electronic Health Record) and HIE (Health Information Exchange) through HPSJ that would be exclusive to my HPSJ patients. These could include disease registries; data exchange with hospitals; and pharmacy, claims and lab data availability.
  8. I am interested in information/assistance with E-mail access (Relayhealth) and other member engagement tools exclusive to my HPSJ patients.
  9. I am interested in information/assistance with HPSJ’s 24-Hour Advice Nurse.
  10. I could use help with staff training for providing care for SPDs.
  11. I could use help with documentation and billing services for SPDs.
  12. I am interested in learning more about Telehealth Options, e.g. caring for patients using home monitors.
  13. I am interested in expanding my practice capability to provide a full medical home, e.g. expanded primary care team and purchasing EHR meeting the meaningful use criteria.
