Effective Date:	June 1, 2010

This is a reminder that coverage under Healthy Connections will end at 11:59 p.m. on May 31, 2010.

Any authorizations you may have for services scheduled after May 31, 2010 will be cancelled.

Any healthcare services rendered after May 31, 2010 will not be covered.

Healthy Connections members have been notified of this change and given information regarding where to obtain information about other coverage options.
